The Social Security Disability page provides information regarding any Social Security Disability application that may have been filed on behalf of a plaintiff. It is intended to be used by the firm representing the plaintiff in connection with the application process for Social Security Disability.

General Information

  1. Select EDIT.

  2. Select the Date Last Worked.

  3. Select the AOD (Alleged Onset Date.).

  4. Select the Date Last Insured.

  5. Select the Amended AOD.

  6. If applicable, place a checkmark in the Previously Denied checkbox.

  7. Select a Previous Denial Date.

  8. Select from the Request for Reopening radio buttons.

  9. Enter any relevant Comments.

  10. Select SAVE.

  11. Otherwise, select CANCEL to return to the previous screen.

Initial Application

  1. Select EDIT.

  2. Select the Filed Date.

  3. Select the Filing Mode.

  4. Select the Decision Date. If applicable, place a checkmark in the Critical Deadlines checkbox.

  5. Select from the Decision radio buttons.
    Selecting either Partial Approval or Denial will open a new panel Request for Reconsideration.

  6. Select the Auxiliary Form Submission Date.

  7. Select the ADL Received Date.

  8. Select the ADL Submitted Date.

  9. Enter any relevant Comments.

  10. Select SAVE.

  11. Otherwise, select CANCEL to return to the previous screen.

 

Request for Reconsideration

  1. Select EDIT.

  2. Select the Filed Date.

  3. Select the Filing Mode.

  4. Select the Request for Good Cause Date.

  5. Select the Decision Date.

  6. Select from the Decision radio buttons.
    Selecting either Partial Approval or Denial will open a new panel Request for Hearing.

  7. Enter any relevant Comments.

  8. Select SAVE.

  9. Otherwise, select CANCEL to return to the previous screen.

Request for Hearing

  1. Select EDIT.

  2. Select the Filed Date.

  3. Select the Filing Mode.

  4. Select the Acknowledgment Date.

  5. If applicable, place a checkmark in the Fee Approved checkbox.

  6. Select a Decision Date. If applicable, place a checkmark in the Critical Deadlines checkbox.

  7. Select from the Decision radio buttons.
    Selecting either Partially Favorable or Unfavorable will open a new panel Appeal to Appeals Council.

  8. If applicable, place a checkmark in the Post Hearing checkbox.

  9. Select a Post Hearing Due Date. If applicable, place a checkmark in the Critical Deadlines checkbox.

  10. Select a Hearing Date.

  11. Select a Supplemental Hearing Date.

  12. Select a Court.

  13. Enter any relevant Comments.

  14. Select SAVE.

  15. Otherwise, select CANCEL to return to the previous screen.

Appeal to Appeals Council

  1. Select EDIT.

  2. Select the Filed Date.

  3. Select the Filing Mode.

  4. Select the Acknowledgment Date.

  5. Select the Submission Date.

  6. Select the Decision Date.

  7. Select from the Decision radio buttons.
    Selecting either Remand or Denial will open a new panel Federal.

  8. Enter any relevant Comments.

  9. Select SAVE.

  10. Otherwise, select CANCEL to return to the previous screen.

Federal

  1. Select EDIT.

  2. Select the Summons/Complaint Filed Date.

  3. Select the Brief Due Date.

  4. Select the Brief Submitted Date.

  5. Enter any relevant Comments.

  6. Select SAVE.

  7. Otherwise, select CANCEL to return to the previous screen.
