Adding Medical Providers

About Adding Medical Providers

The Medical Providers tab is where you enter the details about the plaintiff’s medical providers.

To add Medical Providers:

  1. Select Add to display the fields below.

  2. Select the Plaintiff from the dropdown menu.

  3. Select the Provider from the dropdown menu.

  4. Select an Individual from the dropdown menu.
    For example, an Individual might be someone who treated the plaintiff at a medical office or hospital.

  5. Enter a Start Date from the dropdown menu.
    The first date of treatment with this medical provider.

  6. Enter the End Date from the dropdown menu.
    The last date of treatment with this medical provider.

  7. Enter the Total Visits.
    This field indicates the total number of treatment visits the plaintiff had with this medical provider. Enter a number.

  8. Enter information into the Custom Field.
    This field is free text and can be added by your Admin.

  9. Enter any relevant Comments.

  10. Once all the information for medical providers is entered, select SAVE. Otherwise, select CANCEL.

  11. After selecting the SAVE button, the options for Edit and Delete will appear.
